State Resources (Indiana)
Home and Community-Based Service Waivers (HCBS) - expands Medicaid coverage and allows members to stay in their homes and avoid institutional placement, at no cost to them. This program provides waivers for the aged and disabled, community integration and habilitation, family support, and traumatic brain injuries.
Housing Choice Voucher Program - Provides rental assistance to very low-income families, the elderly, and persons with disabilities so they can afford decent, safe, and sanitary housing in the private market.
Indiana's Community and Home Options to Institutional Care for the Elderly and Disabled CHOICE - a resource that eligible individuals may use to receive support in their homes including: case management, transportation, attendant care, home delivered meals, homemaker services, various in-home therapies, respite care, access to adult day programs in the community, medical supplies, and other services necessary to prevent residential placement into nursing homes, group homes, state hospitals, and other large living facilities.
INvestABLE Indiana - gives those with disabilities the ability to save money in a secure saving's account (up to $100,000) while preserving their SSI and Medicaid benefits.
Program of All-Inclusive Care to the Elderly (PACE) - provides quality based care for Indiana Health Coverage Programs (IHCP) members who are 55 years or older, are certified by the state to qualify for nursing home level of care, are able to live safely in the community at the time of enrollment, and live in a PACE service area. PACE benefits include: primary care services, hospice care, medical specialty services, prescription drugs, nursing home care, emergency services, home care, physical, occupational, and recreational therapy, adult day services, meals, dentistry, nutritional counseling, social services, laboratory and x-ray services, social work counseling, and transportation.
Shelter Plus Care / Permanent Supportive Housing for Persons with Disabilities - provides rental assistance in connection with supportive services to individuals and families experiencing homelessness.
State Resources (Michigan)
811 Project Assistance Rental Assistance (811PRA) Program - allows persons with disabilities to live as independently as possible. The program is intended to support people with disabilities that qualify for Medicaid in their transition out of nursing facilities, out of foster care, out of homelessness, or to maintain their presence in the community. The MSHDA is currently partnering with MDHHS to administer this program.

Michigan Legal Help, State Disability Assistance - the State Disability Assistance program provides cash benefits to people living with a disability, who are 65 or older, or who take care of someone who is differently abled.
Project Based Voucher (PBV) Lists - are attached to specific housing units that are determined to be safe and sanitary through the Housing Quality Standards Inspection.

The Social Security Administration (SSA) - provides monthly payments to people who have a disability that stops or limits their ability to work.
